Ensuring Coverage for the Most Vulnerable

A majority of children in the child welfare system maintain stable health coverage, finds a first-of-its-kind study published in the current issue of the American Journal of Public Health. Ramesh Raghavan, M.D., Ph.D., assistant professor of social work and psychiatry at Washington University in St. Louis and lead author of the study, says that these findings are “a testament to the success of policies directed at securing stable insurance coverage for children.

“Given this vulnerable population’s dependence on Medicaid, protection of existing entitlements to Medicaid is essential to preserve their stable insurance coverage,” he says.

Raghavan says that this type of study is important because it helps measure the effectiveness of current Medicaid policy.

He notes that although the stability of health insurance coverage over time was encouraging, “this dependence on Medicaid may be perilous for children in the welfare system.”

Raghavan cites a number of challenges in maintaining Medicaid coverage for child welfare populations:

-In fiscal year 2007, child health insurance programs faced a cumulative shortfall of $ 800 million across 17 states.

-Medicaid’s new premium and cost sharing requirements

“Children receiving child welfare services may need to pay for non-preferred drugs and emergency department use,” Raghavan says. “It’s also unclear who will pay for health care when kids are removed from homes and placed into foster care.”

-Potential changes to Medicaid-reimbursable case management

“These changes, if enacted, may reduce the flexibility and array of services needed to comprehensively serve these kids,” he says.

Raghavan and colleagues conducted this study using data from four waves of the National Survey of Child and Adolescent Well-Being to examine coverage among 2,501 youths in the child welfare system.

He led a team of researchers from Rady Children’s Hospital, San Diego; University of California at San Diego; San Diego State University; and Tufts University-New England Medical Center, Boston, MA. The HHS/Administration for Children and Families funded their study.

The Areas Most Fragile Babies Have a New Home

Twenty-eight fragile newborns have a new home at Childrens National Medical Center. The 54-bed, state-of-the-art Neonatal Intensive Care Unit (NICU) opened Sunday, May 17, when the babies were transported from the existing NICU. At 35,000 square feet, the new NICU is approximately four times larger than the old unit.

This is a dream come true for our staff, patients, and families, said Billie L. Short, MD, Chief of Neonatology at Childrens National. This NICU was designed with input from our families, nurses, and medical care team members, and we couldnt be more proud of this unit.

The 54-bed NICU at Childrens National is the only Level IIIC NICU in the region, the highest distinction available by the American Academy of Pediatrics. That ranking is conferred on NICUs that, among other cutting edge services, offer ECMO (extra corporeal membrane oxygenation or heart/lung bypass).

CBM 6 Months on – Disabled Children Most Vulnerable in Haiti

CBM says that children with disability are particularly vulnerable in emergency situations and require focused protection measures. Access to non formal education and child centered spaces is particularly critical for children with disability and children who are injured, as they are likely to be left out of relief efforts. Children with and without disability have gone through a highly traumatic experience and are at risk in developing psychological trauma.

When the earthquake struck Port Au Prince, CBM immediately mobilised to provide support to children and adults with disabilities through its partners on the ground in Haiti through the establishment of a country coordinating office for emergency relief and through coordination with other national and international organisations and bodies focused on emergency response, vulnerable populations and disability.

CBM, with the help of other international supporters, set out to establish 10 Child Day Care Centres (CDCCs) to address the needs of children – especially children with disabilities – in post quake Port Au Prince. The children in these day care centres have access to a safe place, non formal education, play, therapy and follow-up care based on individual needs, and are referred as appropriate for more complex medical needs. While access to therapy and assistive devices for children with physical disabilities is clearly needed, other needs are often less visible but equally important. For example, children with epilepsy need access to medication. Supply of medication was affected by the extensive infrastructure damage caused by the earthquake.

Staff from CBMs partner the Center for Special Education (CES) work in these centers and oversee the day to day monitoring and management of the centers. CBMs country coordinating office provides technical and logistic support as appropriate.

Parents report that their children had missed the interaction with the other children in the weeks following the earthquake and being able to attend the CDCCs is bringing a sense of normalcy and happiness back to their lives. Parents appreciate the fact that there is a safe, supportive and loving environment for their children.

CBMs advisers in the areas of mental health and physical disabilities conducted support visits and prepared recommendations for next steps. These recommendations take into account best practice in the field, local staff input and key stakeholder input.

CBM, the overseas disability charity, believe it is vital for disabled people to have the same opportunities as non disabled people. Knowledge of the gang and their methodology is the first step towards curtailing their illegal activities.

In Congress fails to act, many deductions and tax credits will also be reduced or cancelled in their entirety in 2013. These include education deductions / credits, the child tax credit, medical expense deductions, qualified mortgage insurance premiums, state and local sales taxes and energy credits.

There is really no way to know what Washington will accomplish in the coming weeks and fear about the upcoming tax increases is paralyzing business and investment. Many republicans are set against any tax increase, while President Obama insists that any deal has to include tax increases for the wealthy.

The best strategy to minimize your tax bill for 2013 is to continue to exhibit behaviors that have been consistently encouraged through legislation, says Podraza.
